The Sugihara Foundation - Diplomats for Life is a non-governmental organization and is heavily dependent on external funding. Your support is crucial to the Museum's goals - Diplomats for Life's mission to support the work of the Chiune Sugihara. The Museum's work is essential to the Museum's mission of preserving and promoting the memory of Sugihara and Zwartendijk around the world.
1.2% of personal income tax
You can always contribute 1.2 percent of the personal income tax (PIT) you have already paid. This can be done by filling in and submitting the FR0512 v.3 application form ("Sugihara Foundation - Diplomats for Life", company code 110078468) on the deklaravimas.vmi.lt portal. You can find the application form and instructions online, on the website of the State Tax Office, or by following these points:

Rate of tax share: 1.2
You allocate your share of the tax up to the tax period: 2022
Purpose of the tax instalment: for the Sugihara House Museum (or you can leave the box blank)
Check that the data has been entered correctly and process the application.
Once the request has been created, don't forget to click the SUBMIT button.
You can apply for your 1.2% GPT for a period of five years - enter 2027 in box E5 of the application form.
If you wish to amend a previous application for your 1.2% GPT, the latest application will cancel the previous ones. Your application for the 1.2% GPT this year must be submitted no later May 2.
